We provide home and community based services including, but not limited to skilled nursing, personal care, and in-home and community support to adults, and seniors insured thru PA Medical Assistance (Medicaid).

A plan of care is created with an approved health plan member upon admission and updated as needed to ensure that you are receiving appropriate services. The plan of care is based upon problems, needs, and goals identified by your physician’s orders for medication, treatments, safety, nutrition, functional limitations, allergies, permitted activities, mental status, and prognosis. 123 PHHC will utilize information gathered in coordination with your care team during our comprehensive patient assessments care planning process, which involves the identification of patient goals, and nursing interventions that will resolve or alleviate your unique problems and/or needs. We encourage you, your caregiver or your designee to participate in the development of the plan of care and any revisions of the plan.

Our caring staff will keep a watchful eye, dispense daily medications, drive to haircut appointments, safeguard someone unsteady on his or her feet, read aloud, play cards, prepare light meals and snacks, and otherwise function as an extra set of hands, eyes, and feet for your loved one.

 

 

Respite Care Services

123 PHHC provides care and supervision inside the home for young and older adults with physical or mental limitations. Our respite services provide an important option to families who can't afford full-time, in-home care and need some way to keep their loved in a safe, supportive, supervised environment. Respite services also provide family caregivers with much-needed temporary relief, whether for a few hours a week or more extensively.
